Baba’s Payment Assistance Program may cover some or all of the eligible costs connected to your care. This page explains what the program covers, who may qualify, and how to apply.

Approved assistance can be applied to eligible out-of-pocket costs for care provided through Baba.

  • Copays
  • Deductibles
  • Coinsurance
  • Eligible costs associated with Baba services

Eligibility

Who can apply?

You may qualify for payment assistance if both are true:

  • You are receiving eligible care through Baba.
  • Your gross household income is at or below 200% of the current federal poverty level.

Eligibility is reviewed individually and is not guaranteed. We may ask for supporting documents before making a determination.

Application process

How to apply

Get your documents ready

Gather the documents that help us understand your household finances:

  • Pay stubs from the past two months
  • Your most recent federal tax return
  • Proof of government assistance, such as Medicaid, SNAP, or SSI
  • Recent bank statements

Complete the application

Fill in the payment-assistance form and attach the documents that apply to your situation.
